Bookings and payment

All bookings and purchases are subject to availability and payment of the stated fees. You must provide accurate and complete information when booking.

Cancellations and refunds

Consultations may be rescheduled with at least 48 hours’ notice. Late cancellations or missed appointments may be charged in full.

Refunds may not be available once work has started, digital materials have been accessed, or a DNA testing kit has been ordered or submitted, particularly where the service is personalised.

Nothing in these Terms affects your statutory consumer rights. Consumer terms must be fair and transparent under UK law.

DNA testing

Where DNA testing is purchased, you acknowledge that:

• Testing is for nutrigenomics and wellbeing purposes only

• Consent is required

• Relevant information may be shared with LifeCode GX to deliver the service

• Cancellation and refund rights may be limited once the testing process has begun

DNA Testing Information

If your package includes DNA testing, this page explains how that part of the process works.

What the test is for

The DNA test is used to provide nutrigenomics and wellbeing insight. It is not a diagnostic medical test and is not intended to diagnose, treat, cure, or prevent disease.

How it works

Your practitioner places the order and a DNA test kit is sent to you. The test is completed at home using a simple cheek swab.

LifeCode GX states that the sample must be returned with the signed consent form and that the sample cannot be processed without it.

Important points

• The cheek swab is non-invasive

• You must complete and sign the required consent documentation

• Your sample must be returned with the signed LifeCode GX consent form

• Your sample cannot be processed without that signed consent form

• Additional reports may be added for up to 6 months from receipt of the original sample; after that, a new sample is required

  
  

How your information is used

If you proceed with DNA testing, relevant personal, health and genetic information will be used to arrange the test, administer the sample process, obtain the report and provide practitioner interpretation.

Depending on the stage of the process, Chetna Chudasama and LifeCode GX may act as separate independent controllers of personal data.
